If your child is enrolled in another academic institution please complete the Transfer of Student Records form (included in the New Student Enrollment Packet). This form needs to be returned to the current school your child attends.
Permission to Administer Medication – Requires doctor signature for prescription medications.
Special Dietary Needs/Other Allergies – Requires doctor signature.
